SED Punjab letter of 20 October 2026 directing B-Form entry and correction of students in the School Information System
Letter No. DG(M&E)/SIS/2026 dated 20 October 2026, issued by the Director General (Monitoring & Evaluation), School Education Department Punjab.

3.What the letter says

During a review of SIS data, the department found that B-Form numbers of many students had not been entered in the system. District-wise and school-wise lists (List I and List II) are attached with the letter. It gives three instructions:

3.1.Enter every B-Form

Principals must enter the B-Form numbers of all children already admitted. No child whose B-Form has been obtained should be left without an entry. The Principal, District Education Officer and CEO (DEA) are held solely responsible. Enter it correctly

No incorrect B-Form number may be entered, and all entries must be rechecked. B-Forms will be verified with NADRA. For a wrong entry, the School Head will be responsible, and any fine or fee payable to NADRA will be paid by the principal personally.

3.2.No B-Form? Use the father's CNIC

Where a student does not have a B-Form, the father's CNIC number is to be entered in SIS. No child is to be refused admission because a B-Form is not available. CEOs (DEAs) have been told to issue directions to their principals and DEOs at once and to complete the entries within 10 days, making sure no child is left out. The letter is marked Top Priority.

4.Deadline

The letter says "within 10 days". Counted from the letter date, that is around 30 October 2026. Follow the exact date your CEO or DEO office gives.

5.What principals should do now

  • Get your school's entry from List I / List II at your DEO or CEO office.
  • Match the admission register with the B-Form copies in student files. - Collect B-Form copies from parents where they are missing.
  • Check each 13-digit number twice before saving. A second staff member can recheck it.
  • Enter the father's CNIC for students who have no B-Form.
  • Keep copies of the documents you used for each entry.
  • Do not refuse or cancel any admission because a B-Form is missing; guide parents to get one from NADRA.

6.Why accuracy matters

Every entry will be checked against NADRA records. A wrong number makes the School Head responsible, and any NADRA fine or fee will be paid by the principal personally, not by the department.
